The biggest downfall of most places is giving an overly dry cake (think hard baked cinnamon rolls left in an oven too long) with icing that is pretty much just hard royal icing dyed in our "colors"
The best ones I've had are always soft, moist, and filled. Lafayette, Louisiana has some delicious bakeries that do it justice. Poupart's in Lafayette, LA ( http://www.poupartsbakery.com/ ) has some amazing ones.
Meche's Donut King ( http://www.facebook.com/note.php?note_id=176139222422436 they ARE a chain) is another good place to invest in a good quality King Cake. There are tons others but those two are my favorite destinations.
My favorite fillings are strawberries and cream cheese (or cherry.), cherry, strawberry, chocolate, or cream cheese. In that order
